It can be used under the tongue, in the cheek, by injection, as a skin patch, or as an implant. For opioid use disorder, it is typically started when withdrawal symptoms have begun and for the first two days of treatment under direct observation of a health care provider. The combination formulation of buprenorphine/naloxone (Suboxone) is recommended to discourage misuse by injection.Maximum pain relief is generally within an hour with effects up to 24 hours.
Method of administration and dosage of the drug SUBUTEX
Treatment should be done under the supervision of a doctor who specializes in the management of opiate dependence / addiction.
It is recommended that treatment with buprenorphine be prescribed as part of an overall management of opioid dependence.
The outcome of the treatment depends, on the one hand, on the prescribed dosage and, on the other hand, on the associated medico-psychological and socio-educational measures for monitoring patients.
Precautions to be taken before induction of treatment
Before initiating treatment, the doctor should take into account the type of opioid dependence (long-acting or short-acting opioids), the time interval since the last opioid intake, and the level of dependence on opioids. In order to avoid precipitating the onset of withdrawal syndrome, induction of buprenorphine treatment should be carried out as soon as objective and obvious signs of withdrawal appear (demonstrated, for example, by a score indicating slight withdrawal to moderate on the validated clinical scale of opioid withdrawal symptoms (COWS)).
In patients dependent on heroin or short-acting opioids, the first dose of buprenorphine should be taken at the onset of the first signs of withdrawal, but should be taken at least 6 hours after the last opioid intake .
In patients receiving methadone, the methadone dose should be reduced to a maximum dosage of 30 mg / day before starting treatment with buprenorphine. The long half-life of methadone should be taken into account when initiating treatment with buprenorphine. The first dose of buprenorphine should only be taken when the first signs of withdrawal appear and generally not less than 24 hours after the last methadone intake. Buprenorphine may precipitate the onset of withdrawal symptoms in methadone-dependent patients.
Treatment setup (induction):
The recommended starting dose for adults and adolescents over 15 years of age is 2 to 4 mg once daily. An additional dose of 2 to 4 mg may be administered on the first day depending on the individual needs of the patient. During the treatment initiation phase, it is recommended to monitor its administration daily to ensure that the tablet is placed correctly under the tongue and to observe the patient’s response to treatment, which will effectively adapt the dose administered according to the clinical effect obtained in the patient.
Dose adjustment and maintenance treatment:
Following the induction of treatment on day 1, the patient must be stabilized at a maintenance dose over the next few days, gradually adjusting the dose administered according to the clinical effect obtained in the patient. The adjustment of the dosage in steps of 2 to 8 mg of buprenorphine depends on the reassessment of the clinical and psychological state of the patient and must not exceed the maximum dosage of 24 mg per day of buprenorphine.
Daily delivery of buprenorphine is recommended, particularly during the treatment initiation period. Thereafter and after stabilization of his condition, quantities of medication for several days of treatment may be given to the patient. It is recommended, however, to limit the amount of the drug delivered at one time to no more than 7 days.
